The patient is now in retention phase with clear thermoplastic retainers. Upper arch 25 out of 33 aligners and lower arch 25 out of 25 aligners. No refinement CASE 2 This teen patient presents also wanting to improve her smile aesthetics. She is a teen case (14 years of age) and does not want fixed orthodontics. She presents with a significant excess maxilla producing a gummy smile from tooth 14 to 24 of about 3mm. The patient also has an upper midline coinciding with her facial midline, with a thin upper lip thickness. The patient would like to improve her gummy smile with minimal restorative involvement as possible. The patient has a dental class I relationship, bilaterally with an overjet of 4mm and overbite of 80%. This patient presents with healthy gingival tissue biotype of medium thickness with no signs of local or general gingivitis and / or periodontitis. There are also no signs of any restorations or risk or active tooth decay. After 18 months of fortnightly (39 upper and lower) and then weekly change overs of two refinement aligners (upper 5 then 9 and lower 5 then 9) the result was achieved by masking the gummy smile and broaden smile and balancing the buccal corridors. The patient is now in retention phase with clear thermoplastic retainers. The overjet was reduced to 1.5mm and the overbite finished at 30%. There was no need for any restorative and or soft tissue augmentation with anterior upper intrusion very from 1.5mm to 2.9mm achieved in reducing gummy smile from 13 to 23.
