24 AUSTRALASIAN DENTIST EVENTS There is a particular frustration familiar to many implant-placing dentists. The osseointegration is successful, the bone support is sound – and yet the soft tissue keeps undermining the result. Recession encroaches. Peri-implant sites fail to stabilise. The outcome that looked promising at the six-month mark begins to look less convincing at two years. It is a frustration that Dr Jess Liu, periodontist and implant surgeon at Boston University, has spent his career studying. And it is the precise problem that the Australasian Implant Academy’s Fellowship 4 – The Soft Tissue Blueprint – has been designed to address. “As many implant clinicians would say: ‘the bone sets the tone and the tissue is the issue’,” Dr Liu explains. “Soft tissue is not the finishing touch of treatment. It is the foundation of long-term success. Whether around natural teeth or dental implants, healthy, stable soft tissue is essential for function, aesthetics, patient comfort, and long-term stability.” A discipline of its own Soft tissue management is frequently taught as a module within a broader implant course – sandwiched, as Dr Liu puts it, between implant placement and prosthetic planning. Fellowship 4 takes the opposite approach: five dedicated days in which soft tissue surgery is the entire subject. That depth is deliberate. “Soft tissue surgery is genuinely its own discipline,” Dr The tissue is the issue For dentists who have mastered implant placement but still struggle with soft tissue outcomes, Fellowship 4 offers something most courses don’t: a framework for thinking, not just a toolkit of techniques. By Danny Chan Liu says. “It has its own anatomy, its own biology, and its own failure modes. You can’t compress that into a module. When we have five full days, we can actually slow down and teach clinicians how to think through these cases, not just what to do.” The curriculum reflects this. Participants work through diagnosis and classification of soft tissue defects, biotype assessment, keratinised tissue evaluation, flap design, graft selection, suturing technique, periimplant management, and complication handling. The comparison of coronally advanced flaps, tunnelling approaches, and VISTA-style techniques is covered in detail, as is the timing of soft tissue intervention – before implant placement, at second stage, or post-restoration. But the course’s central offering is not a technique manual. It is a decision-making framework that participants can return to practice with and apply immediately. The misconception most dentists carry Dr Liu is direct about where he believes soft tissue education has historically fallen short. “I think the biggest misconception is that soft tissue surgery is primarily about surgical skill,” he says. “Early in my career, I was focused on mastering techniques. What humbled me over time was realising that technical execution is only one part of the equation. Even the most well-executed surgery can produce disappointing results if the diagnosis, treatment plan, or biological considerations are overlooked.” That reorientation – from technique mastery to biological understanding – is the philosophical core of Fellowship 4. Dr Liu teaches a four-part diagnostic filter that he applies to every case: what is the patient’s chief concern; what is the underlying biological problem; what are the anatomical limitations; and what is the most predictable solution for this specific situation. “Most soft tissue cases aren’t difficult because they’re technically demanding,” he says. “They’re difficult because clinicians aren’t sure which approach is most appropriate. That’s the gap the framework is designed to close.” Who is teaching it Dr Liu’s credentials in this field are extensive. He holds his DDS from New York University and completed speciality training in Periodontology and a Master’s degree in Oral Biology at Boston University, where he is now Clinical Associate Professor and Director of the Implant Fellowship Programme. He is a Diplomate of the American Board of Periodontology and maintains a private practice limited to periodontics and implant surgery in Boston.
